- Request for use of the Cathedral Facilities
  
  Before filling out this form, please FIRST confirm your event with the Parish Office by calling (509) 358-4290. If you have not previously spoken with the office, you must call us to check that the date & space you are requesting is available.
  
  Every facilities request form must also be accompanied by a set-up diagram. Blank diagrams are located on the right side of this page, under "Documents". You can mail this or drop it off at the Parish Office at the Ribble Center, 1115 W. Riverside Ave. Spokane, WA 99201. You may also scan and email the diagram to [email protected]. Your facilities request is not complete without this form being turned in!
  
  Please remember, requests for the use of our facilities must completed at least two months prior to the event, and set-up diagrams are due a minimum of two weeks prior to the event.
